Help Using MS WORD fillable forms

The following forms have been developed in MS WORD for AKC Field Event Entries:

• ESS FT Revised 2008 AKC form     RIGHT CLICK AND SAVE
• COCKER FT Revised 2008 AKC form     RIGHT CLICK AND SAVE
• SPANIEL HT Revised 2008 AKC form     RIGHT CLICK AND SAVE

How to use these forms

With a recent version of MS word you should be able to download these forms and TAB from box to box, then print. Note: you should save these forms on your PC (right click on link and SAVE); they will not print properly if simply viewed in most browsers.

They have been formatted to the standard 8.5" x 5" catalog size specified by the AKC. Margins have been pushed to the extreme, and may not print out on some printers.

You may simply print the form and fill it in by hand. The gray boxes will not print.

How to Customize

Clubs may wish to pre-fill the information on the top. To do so, in MS WORD, Click [TOOLS][UNPROTECT DOCUMENT] and you may edit. Form Boxes may be deleted. When finished editing click [TOOLS][PROTECT DOCUMENT][FORMS] (no password) and then save the file.

Please note that most of the fillable boxes have been located by using TABS. If you eliminate the TABS and insert spaces, when the form is filled the boxes may move or line wrap. Be careful!

Pre-Fill Your Information

Individuals might want to pre-fill in information so they use the same form for multiple trials. Here's How:

In MS WORD, Click [TOOLS][UNPROTECT DOCUMENT] and you may edit. Form Boxes should be deleted with the permanent text. Bold Font is suggested. If you are really good, you can place a scanned image of your signature on the signature line and then e-mail the signed form to the event secretary. When finished editing click [TOOLS][PROTECT DOCUMENT][FORMS] (no password) and then the file with a new name like fido-entry.doc

If you do not eliminate the form boxes, but instead place text in the boxes, when you go back to protect the document, the text will be lost.

Use by others

Permission is granted for others to copy, post and distribute the form. The fillable forms were developed by smythwicks.org for the use by the M-AHSC.
